Many clients new to non-surgical facial treatments are confused by the different options available. It's no surprise really when we can open any magazine or newspaper and read how certain celebrities have gone overboard on the Botox or may be sporting a trout pout. These articles are often written by journalists who have very little knowledge of the difference in aesthetic treatments. So how do we determine whether we treat with filler or a botulinum toxin treatment (Botox)?
Typically, lines that develop in the upper third of the face are caused by muscle contraction such as frown lines, horizontal forehead lines and crows feet around the eyes. The first line of treatment for these dynamic lines is usually Botox, as this treatment actually works by relaxing the muscle that directly causes the line or wrinkle. However if a line or wrinkle is static, or is present when the muscle is relaxed i.e. not frowning or smiling, then Botox may only soften the line. In which case it is best to offer a dermal filler treatment to literally fill the crease of the wrinkle.
If lines are a problem in the lower two thirds of the face then usually a dermal filler is offered. Dermal fillers are a naturally occuring substance within the skin - hyaluronic acid. As we age the amount of hyaluronic acid in our skin is diminishing explaining why our skin loses volume, plumpness and creases easier! Where there are lines or a loss of volume,dermal fillers can replace this. Common sites for correction include nose to mouth lines, marionette lines (sad mouth), smokers lines and plumping the vermillion border (edge of the lips). Dermal fillers may also be used to replace lost volume in the cheek area.
